Woman died after hospital’s paracetamol prescription error
Paula Hughes died after being admitted to Queen Elizabeth Hospital in London in January 2022 with a fractured arm. The prevention of future deaths report has found that she had been prescribed two paracetamol-containing medications while in hospital causing acute liver failure. The coroner highlighted a lack of professional curiosity at the hospital
